Yes,
I too had this happening.

I flushed my DNS and also renewed it.
Using command prompt and putting these commands in,
one at a time.

ipconfig /flushdns

ipconfig /renew

Restarted Firefox and this seems to have fixed it, so it could have been me.

while using Chrome.
I'm not sure what the cause was,

That's odd that it would do that at all unless the icon was changed I don't see how it would do that on it's own.

I think that the Domain Name Sever underwent a change of some sort?
Perhaps it was doing maintenance?
Still not sure why it happened,
but it wasn't an isolated incident.

I am not using Mozilla anymore here but Chrome.
I'll have a look at flushing things later today after I've had a sleep ;)
ps. It's not happening anywhere else. Only here.

Edit:- Seems any browser it works with. Mine is ok for now.

Not getting icons again, even after whitlisting GC in the ad blockers/privacy checkers. Restarting Firefox doesn't help. I seems I get a "Not authorized" (Error 403) for all icons:

GC also makes me login each time I connect to it.

The weird thing is that icons are OK in Chromium and copying the icon URL from Chromium to FF loads the icon in FF.

I don't have the appropriate admin permissions.
To be able to attempt to correct this issue.
With that said...

This issue has already been brought to Omega's attention.
What he does about this is on him.

This has also happened to me as well.
Flushing the DSN,
using Command Prompt (Windows Users Only of course),
then renewing it.
Solves the issue for me while using Google Chrome.
