This 60-minute lesson (or if you're in a hurry, there is a 5 minute version, too) shows every step of how I made this image, and will enable you to do the same

Have you clicked on the Show Spoiler links? If they are not working, your browser might be blocking them. You're right about the bottom 'Here' link it's not working.

The overall image and effects look really good, but the band on the left arm doesn't look like it is on her arm. I watched the 5 minute version of the tutorial without sound, as I am at work, so I did not hear you, but it just doesn't look natural.
