You can create a 'red/cyan' anaglyph, with the depth map and the original image using 'Tom Kell's' 3d conversion filters.
Other the that, I'm not really sure what your trying to do.

Is this question for me or Photocomix?

What i am trying to do is best explained in my animation.
Also in this tutorial http://www.youtube.com/watch?v=eHxZrDOnQbE
You can skip to 5:20 when depth map is ready and imported to PS.
Lens blur filter and what it's for.

Here is a script that produces something not entirely unlike what you want (at least as I understand it):

The command is added under "Filters->Map->Pseudo Depth Map" and uses the layer's alpha channel to control the amount of focus blurring.

Here is the result of running the filter on your example image with a Defocus Amount of "8":

There somebody discusses about using depthmaps with Gimp. I can't figure if this is what is needed, though. I would use masking myself, but I'm no expert.

Check if this gives new ideas.

Thanks kimppi!
This technique is well known. It involves handmade depth map which is by definition is not very accurate. I used it
for my 3d animation tutorials

@saulgoode Thank you for another excellent script! It seems to get the job done. The only downside is that scheme scripts can't
have the preview window which is essential for this particular task.

Onkels updated Focus blur plugin works fine now (the previous version I had by some reason didn't render depth map correctly) and has a large preview window.
