Since I updated to GIMP 2.8.14, every time I close and open GIMP, the toolbox window is not opened, and today for the first time, instead of the usual Toolbox menu entry in Windows, I had a New Toolbox menu entry, which created a new toolbox without any of my docked windows.

Is there any way to prevent this?

Not sure what might be happening but you could try resetting your options as outlined in this thread.

Also, you could ensure all of your toolbar option tabs are locked in place

By the way, the tab key acts as toggle to hide and restore the toolbox/toolbars. Under the "Windows" menu option, you can check/manipulate the toolbox, docks & recently closed docks.

Short of some kind of corruption, that's about all I can think of, at the moment.

Yes! the Windows > Hide Docks menu entry was checked.... so the toolbox can be shown using the Windows > Toolbox entry, but when restarted the Hide Dock property is used to know if to show the toolbox or not!
I unchecked that property and now the toolbox is shown as soon as I open GIMP!
