Alonte, perhaps Jolie's tutorial, Make your screen look like it got shot, would be helpful.

Not that i am aware of.
You are better off using either brushes or a layer of glass texture over another layer.
I do have Filter Forge and this filter if you would like me to throw something together for you. :)

Filter Forge is a program that you can use to create or edit existing filters.It isn't free.
Users submit to the Filter Forge site (the link you posted), and anyone who owns Filter Forge can download any of 8500 filters available.
