I don't need this one very often but I have found it useful.

A free graphical interface to the Windows version of PDFTK and the similar,
PDFsam
A free open source tool to split and merge pdf documents

You can also open PDF files with GIMP - I'd use at least a 300 px/inch setting for rendering the files.

If you're not sure of the image resolution, simply open the PDF file in Adobe Reader, click on the image, then right click to copy the image to the clipboard. That seems to copy the image at its intrinsic resolution as opposed to its displayed resolution. Then use GIMP to create a new image from the clipboard (File > Create > From ClipBoard -or- Shift+Ctrl+V). You can then edit or use the image in GIMP, or simply save the image as a file (png or jpg as appropriate).

Other programs will also copy images from the clipboard and let you use them.

CUPS also has PDF capability using the Print function (for Linux machines) and it works great. I haven't checked this option for Windows/Macs.

The cool thing about "Free PDF Images Extract" is you can do a whole book in seconds. "PDFTK Builder" and "PDFsam" are different animals altogether they're for merging, rearranging, splitting, etc. They are all helpful depending on your workflow.
